With the highly contagious Omicron variant spreading at unprecedented levels in Illinois and beyond, Casten is the state’s latest politician to contract the virus.

U.S. Rep. Bobby Rush announced he tested positive for the virus Dec. 28. The 75-year-old congressman, who just announced plans to retire after 15 terms, is also fully vaccinated and boosted.

Lt. Gov. Juliana Stratton, also fully vaccinated, announced two days after Rush’s positive test that she had caught the virus.

Gov. J.B. Pritzker said Wednesday he would be working remotely out of an abundance of caution after coming in contact with a state employee who tested positive.

The Illinois Department of Public Health on Friday reported 42,903 new coronavirus cases. Hospitalizations across the state remain near an all-time high, with 7,096 COVID-positive patients.

The vast majority of those are unvaccinated, officials say. While Omicron is causing more breakthrough cases — infections in people who’ve been vaccinated — the vaccines have still proven highly effective at keeping people out of hospitals.

Abstract
Introduction: Evidence is limited on the effectiveness of a fourth vaccine dose against coronavirus disease 2019 (COVID-19) in populations with prior severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infections. We estimated the risk of COVID-19 deaths and SARS-CoV-2 infections according to vaccination status in previously infected individuals in Austria.

Methods: This is a nationwide retrospective observational study. We calculated age and gender adjusted Cox proportional hazard ratios (HRs) of COVID-19 deaths (primary outcome) and SARS-CoV-2 infections (secondary outcome) from 1 November to 31 December 2022, primarily comparing individuals with four versus three vaccine doses. Relative vaccine effectiveness (rVE) was calculated as (1-HR) X 100.

Results: Among 3,986,312 previously infected individuals, 281,291 (7,1%) had four and 1,545,242 (38.8%) had three vaccinations at baseline. We recorded 69 COVID-19 deaths and 89,056 SARS-CoV-2 infections. rVE for four versus three vaccine doses was -24% (95% CI: -120 to 30) against COVID-19 deaths, and 17% (95% CI: 14-19) against SARS-CoV-2 infections. This latter effect rapidly diminished over time and infection risk with four vaccinations was higher compared to less vaccinated individuals during extended follow-up until June 2023. Adjusted HR (95% CI) for all-cause mortality for four versus three vaccinations was 0.79 (0.74-0.85).

Discussion: In previously infected individuals, a fourth vaccination was not associated with COVID-19 death risk, but with transiently reduced risk of SARS-CoV-2 infections and reversal of this effect in longer follow-up. All-cause mortality data suggest healthy vaccinee bias.

he European Medicines Agency (EMA) announced on Feb. 11, 2022 that its Pharmacovigilance Risk Assessment Committee (PRAC) is assessing reports of heavy menstrual bleeding and lack of menstruation associated with COVID-19 vaccines Comirnaty and Spikevax. Previous reports of period disorders were analyzed in the safety summary reports for COVID-19 vaccines approved in the European Union. It was concluded that evidence at the time did not support a causal link between the vaccines and menstrual disorders.

PRAC has requested an in-depth evaluation of available data from spontaneous reporting systems, clinical trials, and published literature after spontaneous reports of menstrual disorders with both vaccines. “At this stage, it is not yet clear whether there is a causal link between the COVID-19 vaccines and the reports of heavy periods or amenorrhea. There is also no evidence to suggest that COVID-19 vaccines affect fertility,” the agency stated in a press release.

Implications of all the available evidence
Myocarditis after mRNA COVID-19 vaccination is rare, but potentially serious. To better understand possible longer term sequalae of myocarditis, continued follow-up is important, particularly for the patients not recovered by at least 90 days since symptom onset. Vaccination remains the most effective way of preventing morbidity and mortality from COVID-19.

Two years later.....how is the follow up going ?

In December, 2020, the US Food and Drug Administration (FDA) issued emergency use authorisations (EUAs) for the Pfizer-BioNTech COVID-19 (BNT162b2) vaccine and the Moderna COVID-19 (mRNA-1273) vaccine. In May, 2021, FDA expanded the EUA for the BNT162b2 vaccine to include adolescents aged 12–15 years. By July, 2022, more than 200 million people in the USA had received two doses of a COVID-19 mRNA vaccine and more than 1500 cases of myocarditis with onset after mRNA COVID-19 vaccination were reported to the Vaccine Adverse Events Reporting System (VAERS). We searched PubMed for articles published up to April 30, 2022, using the keywords “mRNA vaccine” and “myocarditis”, without any language restrictions. Systematic reviews published in 2022 included more than 5299 individuals with myocarditis after mRNA vaccination and suggested the risk was highest in adolescents and young males after a second vaccine dose. Findings from these systematic reviews suggest that most cases of myocarditis after mRNA COVID-19 vaccination have resolution of symptoms at or soon after discharge from a short hospital stay. However, data on medium-term prognoses for adolescents and young adults diagnosed with myocarditis after mRNA COVID-19 vaccination are scarce.
