Conclusion
CDC estimates that influenza was associated with more than 35.5 million illnesses, more than 16.5 million medical visits, 490,600 hospitalizations, and 34,200 deaths during the 2018–2019 influenza season. This burden was similar to estimated burden during the 2012–2013 influenza season1.

First - most cases of the flu do not require hospitalization. And a season is pretty much a year. So those 20k-40k deaths and those hospitalizations are spread out over (most of them) several months.

That means in a "peak month" you might have say about 50k or so hospitalizations (over the entire US). That comes out to actually quite a small number per hospital. (There are just over 6000 of them).

And the reason for more drastic action is without it, this disease would infect tens of millions of people and kill millions.

It is NOT a social experiment - it has been done for a long time in response to pandemics. The scale of this one is so large it becomes a much larger effort.

We’ve now seen the common pattern. A certain region or jurisdiction reports X number of COVID-19 fatalities over a given period. But when the average number of deaths for all causes is compared to these COVID-19 death tolls they are still dramatically higher than the COVID-19 numbers alone can account for. So we see a large number of unexplained deaths that are almost certainly due to the COVID-19 crisis, whether that is people dying of COVID-19 or dying from other causes at higher rates because of the social and medical care disruptions brought in its wake.

This morning TPM Reader SH sent me this article (in English) from the Spanish daily El Pais which shows another example from the Madrid region of Spain. (An earlier example came from the autonomous community (something like a US state) of Castile and Leon.)

The basic numbers. In 2019 they estimate there were 2,394 deaths between March 14th and March 31st in the Madrid region. In 2020 that number was 9,007. So a difference of 6,613. But the number of official COVID-19 deaths during that period was 3,439. So simple math, 3,174 unexplained deaths.

There’s some wobbliness in these numbers. The 2019 data was only available for the whole month of March. So the authors had to pro-rate each day to come up with this subset. The actual numbers may differ slightly. They are also only using the 2019 numbers. That’s not an average, just one year. So perhaps the mortality was uncommonly light. As I said, there’s more wobbliness than when a tighter multi-year average is used. But again, the differential is so great that it’s highly unlikely that this doesn’t represent a vast undercount of the people who died as a result of the COVID-19 crisis, either as a direct result of the disease or because of compromised access to health care.

There are two possible explanations for this.

1) Fauci genuinely got it wrong. If so, he's part of the early screw-up of the feds overall.

2) Fauci was under orders from Trump to play it down. We know that Fauci would get bolder over the next two months, but he may have been worried about being fired or sidelined at that early stage. Trump's willingness to fire those who cross him is very well established.

We also know that a day later, Trump said this in response to a question from a reporter about worries about a pandemic: "No. Not at all. And we have it totally under control. It’s one person coming in from China, and we have it under control. It’s — going to be just fine."

I lean number 1. At his age Fauci does not need a job, already had and would not jeopardize a distinguished medical career, and I would imagine certainly would not jeopardize public health over an order from Trump.

Then you haven't been watching Fauci closely. Early on he bit his lip very frequently, swallowed his tongue. He has been more outspoken recently, which is good, although he has had fewer appearances.

the R0 naught appears to be substantially higher than previously indicated. If true, this implies much wider infection and asymptomatic rate which increases probability of herd immunity and lowers risk of reopening of the global economy.
